To promote positive change in peoples lives by using social work methods, models and tools, with the aims of helping them to be as independent as possible and to manage risks of abuse and neglect. Make professional judgements within legal and policy frameworks. Carry out assessments of care and support needs. Plan how needs will be met and support implementation. Conduct adult safeguarding enquiries and develop safeguarding plans. Key Responsibilities: Manage a caseload of people with care and support needs and carers. Carry out Care Act 2014 compliant assessments. Work with people to plan how their care and support needs will be met. Carry out financial assessments for care services. Provide information about costs, welfare rights and relevant advice. Arrange services provided or arranged by the local authority. Assess and arrange provision of equipment and assistive technology. Carry out reviews of care and support plans. Reassess needs of people with care and support needs. Recognize and act on safeguarding concerns. Undertake safeguarding adults enquiries. Apply Mental Capacity Act 2005 including capacity assessments and best interest decisions. Attend court as witness when required. Work collaboratively to ensure maximum involvement of service users.
